Hospice refers to a coordinated care and support team that offers a variety of solutions tailored to your specific care needs. Hospice care can begin with general home care and progress to a variety of specialty services, all guided by a personalized plan.
Coordinate Care & Support:
Individuals and families dealing with a serious disease can get physical, emotional, and spiritual assistance through hospice care. Patients can benefit from coordinated care and support as it will facilitate services while they experience pain or discomfort. We are dedicated to creating a dignified, pleasant, and tranquil environment for our patients.
Continuous Home Care:
To avoid hospitalization, we can provide acute symptom care within a residence by hospice workers throughout 24 hour/day shifts.
Routine Home Care:
This can be done in a private home, a nursing home, or an assisted living facility.
Inpatient Care:
We can arrange for general inpatient beds at local facilities, as well as special inpatient hospice units, and provide round-the-clock care until the patient is able to return home.
Respite Care:
By admitting the patient to an inpatient setting without meeting the "inpatient" pain and symptom management standards, respite care provides a temporary "respite" for the patient's primary caregiver. This service is only available for a limited time.
